Echo most often happens if the person you're meeting with isn't using headphones. Their mic pics up their speaker audio, so whenever you speak, their mic is picking up that audio and repeating it back to you.
The best solution is to have Non-VR participants use a pair of USB headphones with a mic for the best quality.
